# The Surge of AI in Youth Mental Health: What the Harvard Pilgrim Data Actually Shows

## What the study measured

The non-disclosure finding is arguably the more consequential one. If a clinician does not know that a teenager has been discussing self-harm, medication or a crisis with a general-purpose chatbot, they cannot factor that into a risk assessment — and they cannot correct advice that was wrong. [1]

## Why the shortage is doing the pushing

The demand-side explanation offered by the researchers and by clinicians is access. Separate work from the Harvard Pilgrim Health Care Institute, published in JAMA Pediatrics in February 2026, documented how barriers to mental health care leave many children without treatment — a gap that has nothing to do with technology and everything to do with the availability of clinicians, coverage and referral pathways. Against that backdrop, a chatbot is not competing with a therapist for most young people. It is competing with a waiting list, or with nothing at all. That is why the framing among health officials has shifted from whether AI will be used in youth mental health to what a safe version of it looks like. [1]

## The regulatory picture, honestly stated

One US federal action is documented and easy to verify: in September 2025 the Federal Trade Commission opened a 6(b) inquiry into AI chatbots acting as companions, sending orders to major providers about how they measure, test and mitigate harms to children and teens. ## What a safety framework has to cover

Drawing on the findings above, four requirements follow directly from the data rather than from speculation:

1. Crisis routing. A tool that adolescents use for emotional support will eventually encounter disclosures of self-harm or abuse. Reliable, tested escalation to human help is the minimum bar, not a feature. [1][5]
2. Disclosure by design. Because most young users are not telling parents or clinicians, tools should actively encourage disclosure and make it easy to share a summary with a trusted adult or provider. [1]
3. Clear labelling of what the tool is. Regulated digital therapeutics and general-purpose chatbots look nearly identical on a phone screen; young users cannot be expected to infer the difference. [5]
4. Data protection appropriate to minors. Mental health conversations from a 14-year-old are among the most sensitive data a company can hold, and the FTC inquiry explicitly probes how providers handle it. [5]
